Statutory Declaration in respect of an alleged vehicle offence made under the Oaths and Declarations Act 1957
Complete this form if you wish to transfer the liability of a moving vehicle (red light / safety camera) infringement notice to another driver.
This Statutory Declaration is made in the event someone else using your vehicle when it was allegedly involved in a moving vehicle offence.
Ensure you complete all applicable sections and have the form witnessed by a Justice of the Peace, Solicitor, Registrar/ Deputy Registrar of the Court, an Authorised Member of Police that is ranked Sergeant or above, or any other person authorised by the Minister of Justice to take a statutory declaration.
The declaration is made under the Oaths and Declarations Act 1957 and is subject to the conditions under the Land Transport Act 1998 if a person makes a false declaration.
Please note: You may only use this form to transfer liability for safety camera notices that were issued by 51½ÖÉä. If you wish to transfer liability for safety camera notices issued by another Issuing Authority (e.g. New Zealand Transport Agency) then you must contact that Issuing Authority.
